Every resident will be encouraged to participate in the development of all reports pertaining to him/her
An initial Plan of Care/Assessment report will be completed on each resident within the first thirty days of his placement at FRESH START YOUTH RESIDENCE. A Plan of Care will be developed for the resident at least every 30 days during the first 6 months that the resident resides with Fresh Start and at least every 6 months thereafter. The child shall have input in the development of the Plan of Care and shall be given an opportunity to express his/her views during each review.
The Plan of Care/Assessment report will contain the following information:
A. Response to limits and routines
B. Academic Achievement
C. Health, Hygiene and Grooming
D. Outings
E. Responsibilities
F. Involvement in Plan of Care
G. Family
H. Relationship with staff
I. Relationship with peers
J. Goals and Objectives
K. Report and all appointments forwarded to the resident’s respective social workers
Discharge reports will be completed and submitted to the respective social worker following the child’s discharge from FRESH START YOUTH RESIDENCE. Our discharge report sets out the date of discharge, the reasons for discharge, the wardship status, future casework goals and a summary of the Plan of Care.
